Important Message from the School Nurse

To the parents/guardians of students entering 7th or 12th grade

All 7th grade students are required to receive two vaccines before the start of the 2023/2024 school year. These vaccines are the Tdap and Meningococcal A.

All seniors are required to receive the Meningococcal A prior to the start of the school year. 

The state gives a 14-day grace period for families to provide the school with proper documentation, otherwise students may be potentially excluded from attending. 

The following are accepted by the Ohio Department of Health:

  1. Documentation from a health care provider, health department, or pharmacy stating the date and time the immunization was given.
  2. signed note from the parent/guardian stating a date and time the student is scheduled to receive the vaccine (if after September 13). 
  3. signed letter from the parent/guardian stating a reason why you do not wish to have your child vaccinated. These reasons can include religious beliefs, reason of conscience or medical exemption. If the child has a medical exemption, the letter must be accompanied by a statement signed by a physician.

Vaccines are given by the Belmont County Health Department by appointment, please call (740)695-1202 to schedule. You may also check with your child’s physician or local pharmacies.
